Our Financial Policy
Beginning June 1, 2020 visits at Balance Integrative Health & Wellness will considered ‘out of network’, meaning we are no longer contracted with insurance providers and no longer accepting insurance. Visits will be paid by the patient and the patient will have the opportunity to submit a claim to their insurance, if desired.
Some common considerations to think about as you pursue integrative healthcare:
- Most people have a high deductible insurance plan and have to pay out of pocket until their deductible is met.
- If you have an HSA or Flex spending account, you can use this money on healthcare visits, IV therapy and more.
- There’s no copay. You pay for the visit on the day of your scheduled appointment.
- Any blood work, which is typically ran through your insurance covered lab, will continue to be the same cost.
- For prescription refills, patients must be seen within the past 6 months.

| Forms to be filled outside of office Visit | $50 | 30 min. of time. Letters or other forms that need to be filled out for work or school. If it’s over 30 min. it will be billed accordingly. |
| Well-Child Visit 0-12 months | $150 | 45 min. Visit |
| Well-Child 1-12 years | $150 | 45 min. Visit |
| Well-Child 12-18 years | $200 | 45 min. Visit |
| Adult Yearly Physical – Without acute problems | $250 | 45 min. Visit |
Extra Services
Phone calls, answering messages through the secure Patient Portal, and requests for completion of medical forms are charged at the rate of $15 per 5 provider minutes spent, with a minimum of $15 per call/message/form.
Missed or Canceled Appointments
Appointments are a valuable resource. With the exception of sudden severe weather events that prevent safe travel to appointments, cancellations for new patient appointments must be communicated by phone to Balance Integrative Health & Wellness Reception (563-284-2422) at least 3 days in advance of the appointment time, and cancellations for follow up appointments must be communicated at least 24 hours in advance of the appointment time. Failure to cancel results in a $30 fee.
Insurance-based Option
For those patients that wish to utilize insurance-based healthcare and who would also like a functional medicine provider, Andrea Simmons, a family nurse practitioner at ANEW Health & Wellness will be practicing at our clinic: 4855 Asbury Road, STE #7, Dubuque, Iowa 52002.
