We Accept all Major Insurances!
Our Financial Policy
Office visits at Balance Integrative Health & Wellness are often covered by insurance. Since we are considered a non-traditional clinic, some of the tests and services, as well as the office visit, may not be covered by your insurance. Balance Integrative Health & Wellness will submit each patient’s claims to insurance but any remaining balance owed is the patient’s responsibility. Payment is expected at the time of service but statements will be mailed monthly for remaining balances.
Our Fee Schedule
When paying cash for exam services (ie not submitting to insurance), costs are listed below:
New Patient Initial Visit – $311
Established Patient Follow up Visit – $65-220 (depending on length of visit 15-60 min. appointments)
Phone calls, answering messages through the secure Patient Portal, and requests for completion of medical forms are charged at the rate of $15 per 15 provider minutes spent, with a minimum of $15 per call/message/form.
Missed or Cancelled Appointments
Appointments are a valuable resource. With the exception of sudden severe weather events that prevent safe travel to appointments, cancellations for new patient appointments must be communicated by phone to Balance Integrative Health & Wellness Reception (563-284-2422) at least 3 days in advance of the appointment time, and cancellations for follow up appointments must be communicated at least 24 hours in advance of the appointment time. Failure to cancel results in a $30 fee not reimbursable by insurance.
